The soft tissue and skeletal elements are diffusely enlarged in the affected digit or digits. 2 Most cases appear sporadically without evidence of inheritance. The anomaly is generally unilateral and may affect more than one digit, when the thumb involved, it assumes an abducted and extended attitude. The bony growth continues until physeal closure; however, soft tissue enlargement may continue into adulthood. The enlarged digits stiffen during growth, which further limits function as the enlarged digit or digits often obstruct grasp and pinch. Patients are usually initially evaluated in childhood with indications for surgery based on functional and aesthetic grounds. Macrodactyly’s management is very challenging due to its progressive and diffuse nature preventing complete correction to normal. Surgical options are either to limit ongoing growth, reduce the size of the digit or correct the deviation, as well as amputation. 3 We present very interesting clinical photographs and plain radiographs of a man aged 68 years with right hand macrodactyly of the thumb and index finger which we believe is the longest follow-up of macrodactyly to be reported in the literature. An attempt to correct his index finger deformity, during childhood, resulted in gangrene to the tip and surgical terminalisation of the index finger was undertaken (figure 1). Despite this, he has maintained full use of this dominant hand. He recently sustained a closed traumatic injury to his right hand and plain radiographs were obtained to rule out any fractures (figures 2 and 3). These demonstrate advanced degenerative changes and fusion across the base of the thumb and carpus and he retained his preinjury function with simple conservative measures.

Pollicization: The Concept, Technical Details, and Outcome
Pollicization substitutes a functioning finger for a deficient thumb. The most indication is thumb hypoplasia with absence or instability of the carpometacarpal joint. However, there are additional causes that may negate thumb function, such as trauma, macrodactyly, multi-fingered hand, and a mirror hand. The technique of pollicization represents a consolidation of contributions from surgeons o...
متن کاملKinematic Analyses of the Thumb during Simulated Posteroanterior Glide Mobilization
OBJECTIVE Thumb problems are common in some health professionals such as physical therapists. The purpose of this case-control study is to investigate the influence of clinical experience and different mobilization techniques on the kinematics of the thumb. METHODS Twenty-three participants without exposure to manual techniques (the Novice Group) and fifteen physical therapists with at least ...

Double dislocations of finger joints are rare. An unusual case of a simultaneous Bennett's fracture/dislocation ofthe carpometacarpal joint and a dorsal dislocation of the metacarpophalangeal joint of the same thumb, and the management of this injury are reported. The patient had an excellent functional result.
متن کاملFlexion and extension angles of resting fingers and wrist.
This study determined flexion and extension angles of resting fingers and wrist in terms of forearm posture (neutral, pronation and supination) and shoulder flexion (0°, 45°, 90° and 135°). The participants participated in 12 angle measurements for 16 finger joints and wrist. The finger joints flexed more in supination than in neutral posture and pronation and the thumb flexed more than the oth...
متن کاملPopulation prevalence of ultrasound features of osteoarthritis in the hand, knee and hip at age 63 years: the Newcastle thousand families birth cohort
BACKGROUND Musculoskeletal ultrasound has been found to be more sensitive than radiographs in detecting osteophytes. Our objective was to measure the prevalence of features of osteoarthritis (OA), in the dominant hand, knees and hips using ultrasound, within the Newcastle Thousand Families birth cohort. METHODS Participants were aged 61-63 (mean 63) years. Knee images were scored for presence...
